Happy BFAA Anniversary to Sam Toth!
Today we’re celebrating 11 incredible years with Sam Toth! She began her journey with us as an orthodontic assistant and has grown into an amazing Treatment Coordinator who helps guide patients through the start of their smile journey every day. Her dedication, compassion, and commitment to our patients make our office a better place, and we’re so grateful to have her on our team. We love you Sammy!

How to Enter:
-Current patients may enter their Super Bowl prediction in-office at either our Portage or Hebron locations.
Limit:
-One entry per current patient.
Winner Selection:
-All participants who correctly predict the Super Bowl winner will be placed into one pool.
-One winner will be randomly drawn from that pool.
If No One Guesses Correctly:
-One winner will be randomly selected from all entries.
Winner Announcement:
-The winner will be announced on all of our social media platforms and will be contacted for prize pick up.
